*A credit card is required to book a physician consultation. Your credit card will not be charged until after your consultation. Appointment cancellations less than 24 hours prior to a booked appointment will be assessed a $25 rescheduling fee charged to the credit card on file. Patient No-Shows will be assessed a $50 fee charged to the credit card on file.
