Thank you for requesting an appointment. If you are a new patient to our office, please select a consultation appointment first. Consultations are free, however if you request a consult with same day treatment, in order to discourage "no-shows," we require a pre-payment of $50 for every 30 minutes of time we need to block out for the requested treatment. Pre-payment is non-refundable, but will be applied to the total cost of your services. If you are an existing patient you may request a specific appointment type. A credit card on file is now required to schedule an appointment. Cancellations later than 48 hours prior to a booked appointment will be charged a $50 fee for every 30 minutes of booked appointment time.
