Signing up with our Direct Primary Care Membership is SIMPLE!

1. Select your plan: Individual, Couple, or  Family of Four
2.  Select your enrollment fee: Individual $0, Couple $0, and Family of Four $0
3.  Select provider as Direct Primary Care Membership
4. Then,  Select the earliest date available.  This allows you to get flagged by our wellness team as joining the membership.  This is NOT an appointment. 
5. Complete online registration - THEN Sign documents via email that will be sent to you.

It's than simple. Select plan, select enrollment fee for that plan.
